About the role
The Service Coordinator is responsible for the effective coordination of services to clients in line with their goals and lifestyle, health and financial requirements. The co-ordinator is responsible for engaging with clients on a regular basis to deliver quality servicing.The position is full time 38 hours per week Mon - Fri.
Key Tasks and Responsibilities
* Actively seek new potential consumers and packages
* review information related to consumer
* Monitor waiting lists
* Assessing client needs and determining expected outcomes:
* Review medical records and special needs and specialist referrals
* Review client home and activities including WHS assessment and upgrades
* Review client goals
* Package budget planning
* Ensure quality service is provided to consumer
* Ensure the provision of person-centred care is applied and suitable care workers allocated to services
* Conduct regular reassessments of client needs and service program
* Liaise with medical and rehabilitation providers
* Ensure all documentation required is correctly completed and submitted in a timely manner
* Assist with all business queries and direct calls as required
* Prepare reports and correspondence as directed by the operational manager
* Recommend care worker further development training to management according to care needs where required.
