If so, we'd love to hear from you!Access to learning and development tailored to your roleAn environment with flexible working optionsA working culture which encourages inclusion and diversityFree on-site parkingAccess to GymMain duties of the jobThe Primary Care Nurse will work as part of a multi-disciplinary primary healthcare team delivering general medical services and elements of occupational health to military personnel and entitled civilian personnel.The Primary Care Nurse is responsible for the delivery of appropriate assessment, planning, implementation and evaluation of care needs, medical screening and advice to patients within their professional competence.This role is critical in the delivery of Defence Primary Healthcare.Job responsibilitiesManaging the care of patients with chronic illnesses e.g. Asthma, Diabetes, Coronary heart disease and hypertension ensuring continued maintenance of the Chronic Disease Register.Undertaking technical nursing treatments and procedures within your scope of practice for patients with complex/specialist healthcare needs (e.g. wound care, sutures removal, Echocardiograms, Cytology, Audiometry, Ear Care) at site.Perform and supervise tasks required for the general running and administration of the Treatment Room, ensuring care delivery in accordance with national, Service policy and protocols; whilst matching care provision to the needs of the Service.Have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations making effective use of the electronic Human Resources (eHR) system at both sites.Please refer to the attached Job Description and Person Specification for the full list of duties.Person SpecificationQualificationsProfessional and specialist knowledge in primary healthcare acquired through post graduate degree/diploma or actively working towards this and can demonstrate equivalent level of competence and experience.Learning and Assessing in Practice Qualification or equivalent practice assessors training Quality improvement methodology or willingness to undertake training.Must be in date for Basic Life Support (BLS), Anaphylaxis, Automated External Defibrillation (AED), Infection Prevention and Control (IPC), Safeguarding Level 3 (Children and Adult depending on population at risk) or willingness to undertake training.Registered non-medical prescriber or willingness to undertake training.
