Absolute Home care are actively seeking an experienced and passionate candidate to fill the position of Home Care Package Coordinator.We provide holistic support that empowers older Australians to remain independent in their residence. We collaborate with a multidisciplinary team to ensure safety and appropriate supports are provided to our valued older Australians.For candidate to be successful in the position, strengths should entail:Communication - high standard of communication, negotiation and relationship buildingCollaborative practice - work with professionals to achieve sets goals for older Australians with clear understanding of regulatory bodies guidelines and expectationGoal oriented approach - Support older Australians to reach reasonable set goalsSound understanding of the Home Care Package.Essential criteria:Relevant qualification in social work, case management, community services or similar or equivalent community aged care experience.Thoroughly understand aged care and the risks associated with aging.Demonstrate ability to work with a multidisciplinary team to work towards consumer goals and work independently.Adapt to the strong ethos of Absolute Home Care placing the consumer at the center of care.WWVP, police check, drivers license, willingness for us to complete a insolvency check on your behalf.Evidence the ability to write meaningful constructive care plans and complete home risk assessment with the consumers.Job Tasks and Responsibilities include;-Identification of consumer goals and needs-Development of consumer directed, goal based care plan-Engaging with allied health professionals and service contractors-Establish and monitor consumers HCP budgets in partnership with consumer and/or representative-Liaising with external service providers for care monitoring and documentation-Regular reviewing on ongoing adequate careThe HCP co-ordinator needs to be knowledgeable or aware of the 'Quality and Safety Standards' and will need to contribute to audits performed by the Aged Care Commission.
