What You Will Be Doing The Transition Care Program is seeking an experienced, innovative and flexible Clinical Nurse Consultant (CNC) to work as a member of the TCP CNC team.
The program is jointly funded (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.
The successful applicant will need to be flexible in terms of working within the community environment across the large geographical catchment covered by Peninsula Health.
The CNC will need to demonstrate sound clinical judgement, assessment and practice skills, and have an understanding of the issues affecting patients with complex functional and cognitive needs and an interest in gerontological issues.
The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team.
Please note the following; Flexible working environment with options to do some work from home Monday to Friday Roster- No weekends or evening shifts Opportunity to support older patients transition from inpatient hospital episodes to home with the support of an experienced multi-disciplinary team The opportunity to work with patients through the TCP episode ( 12 weeks) allowing time for relationship building and optimisation of outcomes Great multidisciplinary, close knit team where interdisciplinary working is the norm PD Transition care program -Clinical nurse consultant What You Need We anticipate you will have: • Current registration with AHPRA.
• Demonstrate highly developed clinical assessment, therapeutic and care planning skills.
• Demonstrate knowledge of issues affecting patients with complex care needs.
• Demonstrate a knowledge and understanding of Residential aged Care and the Community service sector and their relationship with the health service.
• Flexibility to work across the different sites of TCP • Demonstrated clinical leadership with the intention to promote professional competence and foster individual development.
• Demonstrated ability to coordinate and participate in quality improvement and evidence based practice activities.
• Excellent communication and interpersonal skills, including negotiation, conflict resolution and problem solving.
• The CNC should be capable of managing a range of clinical conditions.
The successful applicant will need to be highly motivated, innovative and enthusiastic and to be able to work both independently and as part of a team.
