Holistic Horizons is expanding into the Logan area as well as into the Aged Care community space. We are looking for a strong passionate leader to guide the company into this space through accreditation and launch. Position Overview The Care Manager is a key leadership position within the Services division and is primarily responsible for providing clinical leadership and direction to all Aged Care staff across all service deliveries, ensuring that day-to-day clinical work is carried out effectively to meet the needs of our clients. This role contributes to the development of evidence-based and innovative clinical services and will promote the delivery of care that is of the highest quality. The Care Manager will be actively engaged in clinical and caseload reviews and manage referrals and demand for clinical services, providing direct clinical services where required. The Care Manager will proactively support Holistic Horizons community services financial outcomes to enable the team to maximise funding in different service models. The Care Manager will be adept at leading a multi-disciplinary team and be an experienced and innovative health professional with considerable knowledge and experience in aged community-based care. This position will also form and maintain close and effective working relationships with key internal and external stakeholders. This role will suit someone who prefers a hybrid model of mostly working from home during the launch of Aged Care and Disability in Logan and someone who lives in the Logan Area. A car will be supplied as part of this role. Full Salary Packaging as a Public Benevolent Institution is available.
