The role of the Support Worker is to provide support to customers that is flexible and individualized to facilitate customer involvement in capacity building activities and promote community participation and inclusion.
Key Responsibilities:
* Provide individually tailored support to maximize dignity, personal growth, and independence.
* Contribute to developing, following, and implementing programs that support customers in developing skills associated with daily living and to increase their capacity to be a part of the community.
* Perform duties in a manner that promotes the dignity, independence, and acquisition of skills of customers.
* Contribute to the ongoing development of a person-centered team.
* Complete all required paperwork, documentation, and recording as per Tailored 4 Choice policy and procedure, including monitoring and documenting activities and goals as part of their NDIS plan.
* Support customers to identify skills that will enable them to meet goals through an individualized support plan.
* Ensure all customers are treated with respect and dignity and provide culturally respectful and appropriate service delivery.
* Maintain a high level of support in all aspects of customer care.
* Respect privacy and confidentiality of customers in accordance with privacy legislation and Tailored 4 Choice policy and procedures.
* Demonstrate a working knowledge and understanding of the OH&S requirements in the workplace.
* Maintain positive communication and interactions with all stakeholders.
* Ensure timely, accurate, and relevant documentation is maintained within the workplace.
* In conjunction with other members of staff, assist in developing, implementing, and evaluating training and behavior programs to attain identified individual customer goals.
* Actively contribute to a team environment through open communication, participation in regular staff meetings, planning, and reviews.
* Be adaptable to changing circumstances and able to prioritize.
* Display a client-focused and person-centered manner.
* Work effectively in a team environment as well as independently while enjoying a degree of responsibility in your position.

Minimum Qualifications/Experience:
* Certificate III in disability or equivalent (Minimum Requirement).
* Previous experience in disability, aged care, or mental health field is an advantage.
* Experience in working with challenging behaviors.
Compulsory Accreditations:
* NDIS Worker Screening/Cleared police check within the last 12 months.
* Current Working With Children check (WWC).
