Objectives of Health Care in the Province:
* To promote the concept of positive aging to participants of all ages, including a spirituality of aging.
* To promote health, the prevention of disease, and a safe-living environment.
* To provide living arrangements and support structures for aging participants, enabling them to remain in a normal and supportive community setting for as long as possible.
* To encourage participants to find realistic and appropriate ways of continuing involvement in ministry/service to others.
* To encourage participants to see ministry as an integral part of life and to be open to receiving ministry as appropriate, especially in later years.
* To provide support to manage and/or reduce the effects of debilitating disorders.
* To develop a range of residential options that provide choice and appropriate support as participants age.
* To promote health and wellness and proactive strategies for all participants.
Registered Nurse Position Purpose:
To provide continuity and ongoing health care support to participants in the Sydney region, across a 5-day week, in line with the TOCB Model of care.
Key Responsibilities:
Key Accountabilities and Responsibilities:
* Weekly communication and updates on current health issues of the participants in NSW from the Director of Health and Wellbeing.
* Communication of current health issues of the participants with the Participants Cluster team for NSW.
* Visitation during working hours of participants who have health needs that must be monitored and assessed over the weekend. Competency in taking vital signs and interpreting changes in these observations to support participants’ health needs.
* Report to Health Care Coordinator participants’ health needs, changes in physical and psychological needs that may need further assessment and change in delivery of care.
* Accompany unwell participants to the hospital to provide medical staff with an up-to-date report on the participants’ current health issues, including a list of current medication.
* Attend and provide transport to participants who may have medical appointments during working hours.
* Other duties as assigned by the Director of Health and Wellbeing or their delegate(s), e.g., assisting participants who are frail and recently returned from the hospital with personal care needs and medication supervision.
Key Responsibilities with other Health Care Providers:
* Organise My Aged Care assessments as independent participants age.
* Ensure home care packages and CHSP are being utilized to capacity.
* Maintain positive relationships with Aged Care Service Providers to facilitate entry into an Aged Care Facility.
* During hospital admissions for participants, keep regular contact with the doctor, Registered Nurses, and other health care providers to optimize discharge planning.
* Organize aids that will promote health recovery from hospital admission.
Knowledge, Experience, Qualifications, and Skills:
Health Care Worker Skills and Qualifications:
* Degree in Nursing
* Demonstrated experience working in the aged care sector.
* Ability to analyze medical information, vital signs, and data.
* Prior experience in a healthcare setting, where there was a demonstrated ability to work with other employees to achieve agreed goals.
* Understanding of medical confidentiality laws and practices.
* Current registration with the Australian Health Practitioner Regulation Agency as an RN
* Current Driver’s Licence.
* Current First Aid and CPR Certificate.
* Computer skills and mobile phone skills.
* Current Influenza and COVID vaccination status for appropriate age recommendation.
* Excellent verbal and written communication skills.
* Current National Police Check.
* Working with Children check may be required.
