Your new role
The Program Coordinator for Home Care Packages (HCP) plays a vital role in supporting clients to live independently at home and within their community. By providing timely and effective support, you will empower clients to achieve their personal goals, maintain their independence, and enhance their quality of life while ensuring they receive the necessary care.
Key Responsibilities:
1. Deliver person-centred services in line with the organisation's values and compliance standards.
2. Assist clients in maximising their HCP funding.
3. Manage ongoing care, including goal-setting, assessment, and care planning.
4. Foster relationships with staff, volunteers, and health professionals for coordinated care.
5. Maintain accurate records and support continuous quality improvement.
6. Supervise and roster Support Workers.
7. Monitor CHSP clients for early signs of deterioration and transition to HCP as needed.
8. Contribute to the growth of the organisation and manage NDIS Program care.

Qualifications:
1. Qualifications in Aged Care, Community Services, Nursing, Social Work, or Allied Health are highly desirable.
2. Current CPR and First Aid Certificate
3. Current C Class driving licence
4. National Police Check or NDIS clearance
Experience:
1. Knowledge and experience in the Aged Care Sector with case management/Home Care Packages.
2. Commitment to providing exceptional consumer service across all channels - written, phone, and face-to-face.
3. Ability to communicate clearly and concisely, adapting style based on the audience.
4. Excellent attention to detail and written communication skills.
5. Ability to apply knowledge in a practical, commercial manner.
6. Willingness to assist and support team members as required.
7. Effective time management and organisational skills to meet deadlines and carry out administrative duties efficiently.
