About the Role
The primary responsibility of this position is to support residents in enhancing their daily living, social, and emotional abilities. This includes ensuring the well-being of residents in day programs, group homes, and their own homes, as well as promoting their related welfare matters.
Key Responsibilities:
* Provide mental health, social, and emotional support to residents in a residential care setting.
* Collaborate with a team to create various support plans for residents, such as Care Plans, Meal Plans, Activity Plans, and Rosters of Care.
* Assess residents' needs related to health, welfare, employment, housing, training, and other appropriate services for their wellbeing.
* Observe residents for any changes in health and promptly report concerns to appropriate medical personnel or supervisors, including responding to emergencies and following safety protocols.
* Facilitate connections between residents and their family, community, and culture.
* Maintain accurate records of resident care activities, medications administered, and any incidents or observations according to organizational policies.
* Supervise meal planning and preparation based on residents' dietary needs and preferences.
* Communicate effectively with residents, their families, colleagues, and other healthcare professionals to coordinate care and support resident needs.
* Manage budgets and funding for participants for cost effectiveness.
* Liaise with welfare agencies to ensure high levels of support.
Requirements
* A minimum qualification of Diploma in Community Services or equivalent qualification in a related field.
* In the absence of relevant qualifications, three years of relevant experience may substitute for formal qualifications.
* Minimum 1 year of industry experience preferably in a disability/mental health setting/aged care/residential care setting.
Skills and Attributes:
* Establishing and sustaining professional and supportive relationships with clients.
* Thorough comprehension of the NDIS framework and disabilities within the Australian context.
* Possession of up-to-date First Aid certification.
* Accredited in Manual Handling techniques.
* Strong customer service aptitude.
* Outstanding ability to manage time effectively.
* Competent in administrative and case management tasks.
* Excellent verbal and written communication skills.
* Desire and enthusiasm to help others.
* Generous disposition exhibiting endurance, composure, and understanding towards senior citizens/disabled people.
* Exhibited sympathetic comprehension of complicated trauma and its consequences.
* Put the needs of residents first while producing outcomes.
