Key Responsibilities:
* Develop individual care plans to achieve personalized functional outcomes
* Conduct falls prevention, education, and strength training programs
* Collaborate closely with local community teams and stakeholders

Requirements:
* Current registration as an Occupational Therapist with AHPRA
* Prior community experience highly desirable
* Ability to provide high-quality treatment and intervention
* Police check dated within the last 3 years
* Willingness to obtain annual flu vaccinations and Covid-19 vaccinations
* Own vehicle and NSW driver's license
