The Home Therapies CNC will work collaboratively with nursing and medical staff, particularly the Home Therapies Medical Lead, as well as the wider multi-disciplinary team to optimise patient care in a safe, effective, and timely manner, including management of acute and urgent start dialysis.A key priority of this role is to further optimise home therapy options for suitable patients across both PD and HHD. The CNC will have a leadership role within Home Therapies, focusing on clinical services, education, workforce, quality and safety, and research.
