We’re on the lookout for a part-time Health Linker to elevate our mission. As a Health Linker, you’ll collaborate with clients, allied health providers, general practitioners, and community services to streamline access and coordination of essential support that meets the unique needs of each individual.

What You’ll do:
* Deliver holistic, coordinated support to clients engaged in the WARATAH Program, implementing care plans and connecting them to vital health and social services.
* Collaborate with clients to craft personalised support plans that enhance health and well-being, re-establishing connections to community groups and essential services.
* Help individuals tackle broader challenges affecting their health, such as financial difficulties, housing issues, unemployment, isolation, and caregiving responsibilities.
* Provide tailored guidance to clients, their families, and caregivers to boost health literacy and promote healthy lifestyle choices
What You’ll need:
* Relevant tertiary qualifications in Nursing, Aboriginal Health, Allied Health, Community/Human Services, Social Work/Social Science, or equivalent.
* A solid understanding of the broader determinants of health—including social, economic, and environmental factors—and their impact on communities.
* Proven experience in co-developing person-centred support plans that prioritise client well-being and aspirations.
* Advocacy skills to enhance client access to programs and inspire community participation.
* Exceptional communication and interpersonal abilities, particularly with culturally and linguistically diverse (CALD) backgrounds and First Nations communities.
* Experience in partnership building and collaborative work across various organizations to achieve client-focused outcomes
