Band 5 - Salary Range $78,646 to $90,563 per annum + 11.5% Super + ADO

The Opportunity
The Family Support Worker reports to the Family Services Team Leader and sits within the broader Community Care and Active Living business unit.
This role supports the delivery of Council’s Integrated Family Services program, providing outreach support to children, young people and their families with a focus on enhancing parenting capacity and skills, parent-child relationships, child development, and social connectedness.
Working within the Best Interest Case Practise Model, you will develop and maintain effective working relationships with a diverse range of stakeholders including schools, local community agencies and organisations in the delivery of family supports.
Key Responsibilities
* Support the needs of children, young people, and their family within the Best Interest Case Practise Model, including comprehensive assessments of presenting risks and needs, holistic case management, information, and advocacy.
* Actively engage families in the development and implementation of action plans and regular reviews to ensure identified goals and outcomes are achieved, including making of appropriate service referrals.
* Utilise a strengths-based approach, provide case management support to meet the needs of children, young people, and their family, including parenting skills enhancement, modelling appropriate strategies, and routine setting.
* Participate in service development activities and projects, including Continuous Quality Improvement requirements.
* Maintain effective client records via an electronic client management system and complete individual Key Performance Indicators and reporting requirements.
About you
To be successful in this role you will have the following:
* Tertiary qualifications such as social work, welfare work or other relevant human services degree with some relevant experience or a lesser formal qualification with substantial relevant experience working in the family and parenting support sector.
* Experience working within the family and parenting support sector through a client case management model.
* Current valid Victorian Driver’s Licence.
* Must hold or be able to obtain an Employee Working with Children Check.
