With our focus on providing patient-centered care, our Social Workers play a crucial role in supporting patients in hospital settings.You will be joining a multi-disciplinary team on our inpatient ward, working with experienced healthcare professionals to identify patient needs and develop appropriate care plans.
You will be responsible for providing emotional support, counselling, and practical assistance to patients and their families and ensure a smooth discharge.Your duties will include:Provide emotional support, counselling, and practical assistance to patients and their families.Work closely with healthcare professionals to identify patient needs and develop appropriate care plans.Conduct assessments and coordinate appropriate community resources for patients who require ongoing support.Develop and implement discharge plans for patients, ensuring a safe and smooth transition to home or a community setting.Provide education and support to patients and families regarding hospital policies, procedures, and available resources.Your Skills and ExperienceA bachelor's or master's degree in social workRelevant experience working as a Social Worker in a hospital setting, with experience in an acute ward setting.Eligible to be an accredited member of AASW.Demonstrated excellent interpersonal, communication, and problem-solving skills with consumers, their families, and all health care professionals.Ability to work collaboratively within the multidisciplinary team including internal and external agencies.Ability to work independently, flexibly and manage time effectively, prioritize competing demands.Knowledge of the Aged Care sector will be highly regarded.What's in it for you?Professional Advancement: Recognition, Rewards, Professional Development, and Employee referral programs.
