About the role
As a Home and Community Support Worker you will provide support to people in our community living with a disability. The support you provide will allow people to live independently, by assisting and encouraging them to participate in daily tasks, build skills, support and promote daily and community activity participation, and access appropriate services.
Duties include:
Provide support to participants and elderly people with a Physical, Psychosocial and/or Intellectual Disability.
Understand and provide the appropriate care to participants with both high and low needs.
Provide personal care, domestic duties, manual handling, and medication administration.
Provide direct support to each participant in accordance with their NDIS plan, person centred plan, and job service instruction.
Support participants to build capacity.
Work with participants, family/carers, relevant case managers and other professionals to support the participant’s individual goals.
Link participants with appropriate support networks, support to sustain relationships.
Identify and report risks and barriers to service delivery.
Skills And Experiences
Key Selection Criteria
Qualification in either individual support, disabilities, mental health or similar.
Experience as a Support Worker (working with people with a disability preferred)
First Aid and CPR Certificate (to be obtained and maintained at your own expense)
Driver’s license and access to a reliable and insured motor vehicle
Applicants will be required to hold a NDIS Worker Clearance, a Criminal Record Check and Working with Children Check (WWCC).
COVID-19 Vaccination Status will need to verified prior to employment.
