We are seeking applications for full-time, part-time, and casual Community Registered Nurses to join our dynamic community nursing team.
What is involved in the role?
We are looking for dedicated Registered Nurses located in the Perth metropolitan, Mandurah, and Bunbury regions to deliver a high level of nursing care to our elderly and disability clients in their homes.
Our Community Nursing team are actively committed to promoting choice, control, and quality of life for our elderly and disability client through consistent, person-centred care. This role will allow dedicated nurses to use their clinical experience, problem-solving skills, and holistic approach in the community setting. Training and a supportive buddy system is offered to new nurses entering the community sector.
Key duties of this position includes:
* Collaborate with our Community Nurse Manager and engage with stakeholders
* Provide comprehensive nursing care and individualised care planning for community-based clients
* Manage referral network for client admissions to My Flex Health services
* Ensure adherence to quality standards and guidelines
* Support client independence throughout their healthcare journey
* Mentor and educate Multi-Skilled Support Workers
* Communicate respectfully with service users, representatives, staff, and others
* Apply funding regulations in nursing care, care planning, and referrals to Allied Health services
* Participate in quality assurance programs and policy reviews
* Conduct Client Risk and Hazard Assessments to mitigate risks for clients and employees
* Support and educate junior registered nurses
* Assist in coordinating care to ensure safe and individualised healthcare practices for all My Flex Health clients, especially the most vulnerable.

The ideal candidate will be willing to obtain / provide evidence of the following:
* Current AHPRA Nurse Registration with relevant experience in the Community setting during the past 2 years
* NDIS Worker Screening Check*
* National Police Clearance
* NDIS Orientation
* Current CPR
* Manual Handling (In-house Training provided)
* Current Drivers Licence
* Working With Children’s Card
* Demonstrated advanced clinical experience in the Community Disability and/or the Aged Care sector that aligns with best practice standards
* Advanced verbal, written, and interpersonal skills
* Ability to prioritise workload by utilising efficient time management skills, whilst having the ability to undertake multiple tasks within allocated time frames.
* Advanced / Intermediate knowledge of the Aged Care and/or Disability Sectors and their application to the community setting
* Demonstrated ability to work autonomously and within a team environment.
* Highly developed negotiation, problem-solving, and analytical skills.
* Excellent customer service and interpersonal skills with a strong desire to achieve client and staff expectations
* Create and sustain an environment of awareness for continual improvement and risk mitigation
