These multi-disciplinary services are provided in the centre, community, home or via Telehealth. CCHS operates from the Caulfield Hospital site.The RoleWe have an exciting opportunity for a Health Promotion Coordinator (HPC) to join our Community Health Service responsible for leading and coordinating health promotion services across CCHS. The position will work as a member of the Population Health Team and will collaborate strongly with members from other teams within the organisation to undertake high quality priority setting, strategy planning, implementation and evaluation. The HPC will receive support, guidance and direction from the Population Health Team Leader across all areas of responsibility.
