With offices in Orange, Bathurst, Mudgee, Dubbo and Parkes and services reaching the Central, Western and Hunter regions of NSW, our purpose is to achieve healthy, engaged and vibrant communities in which individuals enjoy opportunities to thrive and build a promising future for their families.We are looking for a full time Support Coordinator to join our housing team based in Bathurst.We are looking for someone with strong case management experience to work closely with our tenants to support them to sustain their tenancies. This is a full-time permanent position working 38 hours per week, Monday to Friday.The Support Coordinator is classified as a Level 4.1 under the SCHADS Award, which equates to $85,355 gross per annum. This will increase on an annual basis in line with the pay points in the Award. As Housing Plus is a Not for Profit organisation, we have access to excellent salary sacrifice options to increase your take home pay!The Support Coordinator will:Conduct regular assessments and develop tenant support plans;Make referrals to appropriate support partners;Build positive collaborative partnerships with relevant community support services;Attend interagency meetings;Maintain accurate records and case notes;Proactively work with the Housing team to identify risks and barriers.To be successful, you will require:Minimum 2 years’ experience in community services or client support and intervention;Demonstrable knowledge of case management principles;Knowledge of the community services sector and available services for clients in the Bathurst area;Experience working with vulnerable people and Aboriginal and Torres Strait Islander community members;Ability to develop and maintain productive relationships with key stakeholders including referral partners;Ability to work effectively as part of a team and contribute to team success;High degree of initiative and the drive to achieve set KPIs, high levels of productivity, achievement orientation;Highly process-orientated.You will also need a current driver’s licence, ability to obtain a Police and WWC Check, and intermediate IT skills.In return, we can offer you the chance to make a significant contribution to our organisation and community by driving this new innovative program, and the opportunity to be part of a dedicated team who are passionate about what they do.
