The Eyre and Far North Local Health Network is seeking a compassionate and dedicated Community Support Worker to join our team and provide vital care and assistance to aged, frail, or disabled individuals.

Key Responsibilities: Provide personal care, housekeeping, meal preparation, and social support to clients in line with their care plans.
Foster strong, trusting relationships with clients, their families, and other health professionals.
Monitor client well-being, report changes in health or functionality, and provide diversional therapy as needed.
Assist in the provision of training and contribute to quality improvement initiatives.
Maintain accurate records and complete necessary risk assessments to ensure safe service delivery.
Support clients in achieving their goals under the Consumer Directed Care model, promoting independence and choice.
What We're Looking For: Certificate 3 in Individual Support (Ageing, Disability, or Home and Community).
Current driver's licence and access to a reliable, insured vehicle.
Experience in the provision of a direct care service in a health related field and experience in dealing with aged, frail or disabled persons.
Ability to work independently and as part of a team.
Excellent communication skills and the ability to support people from diverse backgrounds.
Flexibility, patience, and a customer-focused approach to care.
