Shift differentials are also available for nurses opting for evening and overnight shifts. Chestnut is seeking nurses to drive integrated care across various programs. An ideal candidate is available to work shifts at any location including the Health Centers (Granite City, Maryville, and Belleville), Residential (Maryville), or Housing sites (Pontoon Beach, Madison, Fairview Heights, Collinsville). Must be available to work a minimum of two shifts per month, if hired as PRN (as needed). Responsibilities Carry out health care plans designed by practitioners. Administer treatments, medications, and medical tests. Gather patient health histories. Assess patients for acute physical issues. Act as a liaison among prescribers, patients, laboratories, PCPs, other providers, pharmacies, and staff to ensure continuity of care. Provide health teaching as well as support and guidance for patients in maintaining a therapeutic environment.Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness. Qualifications High school diploma or equivalent; practical nurse licensed in Illinois OR registered nurse licensed in Illinois. Requires basic computer skills and experience to include MS Word, email, videoconferencing, e-prescribing, and entry into an electronic medical record (EMR).
