About the role:
* Managing an existing caseload of participants, while facilitating new intakes of participants and participant referrals
* Assisting participants in understanding their NDIS plans, and how to utilise them
* Connecting participants with supports to match their needs, including but not limited to allied health, support workers, mental health and other therapists.
* Liaise with the NDIA planners and officials to build NDIS plans that suit participants' needs.
* Liaise with participants, their families, their guardians, GPs, public trustees, mental health teams and support networks to facilitate the best NDIS supports and informal supports possible.
* Budgeting NDIS plans and funding and allocating funding to supports and organisations.
* Writing end of plan reports and progress reports that will be submitted to the NDIA.
What we are looking for:
* Prior experience in a support coordination role (essential)
* Level 2 Support Coordinator
* Evidenced understanding of the NDIS, including funding allocation, review processes and regulations
* Qualification in Community Services, Social Work, Mental Health or Case Management (highly desirable)
* Computer literacy skills, including capable use of Microsoft Office products
* Strong professional boundaries, time management, task prioritisation, and communication skills
* Demonstrated problem solving skills
* Demonstrated risk assessment capability
* Ability to work both autonomously and as part of a wider team
* Blue Card & NDIS Workers Screening Check, or willingness to obtain.
* Current Driver License & own vehicle
