The Team: Work as part of the Community & Outpatient Social Work Team and the larger Hospitals South Social Work Service Based in Hobart/Kingston hub Potential future vacancies for other sites across Hobart The Role: Within a primary health care framework, provide a social work service to individuals, families, groups and communities with a focus on developing evidence informed services that are reflective of community needs. As a senior member of the health care team provide support to management in developing, implementing and monitoring policies and practices that reflect the priorities and standards of the service. Promote community awareness in relation to building community resilience and work with other agencies, organisations and the wider community to develop an integrated approach to delivering health services. You will need: Extensive experience as a practising social worker, together with substantial knowledge and skills in the provision of a Social Work service in community settings and sound knowledge and understanding of the principles of primary health care and of current state and national health vision and policy. Knowledge of the principles of community development and skills in developing and evaluating health promotion programs, understanding and experience in community recovery assessment and response plus knowledge of community resources and the ability to provide support to health professionals, non-government service providers and the wider community in relation to health care needs. Proven high level communication skills, including the ability to work collaboratively as part of an interdisciplinary health care team and to build positive working relationships with other service providers, local support organisations and the wider community. Proven commitment to quality improvement activities and the ability to undertake and contribute to practice based research and evaluation.
