About The Role:
Are you a passionate leader ready to make a difference in the lives of older Australians? Join our Lake Macquarie/Hunter Home Care team, working collaboratively with the Home Care Manager to lead our dedicated team. Based at our Cooranbong office, this full-time role offers the opportunity to guide and support excellence in Home Care services and ensure compliance with Aged Care Standards.
What you’ll Do:
* Work collaboratively, leading remote teams to deliver personalised, flexible and holistic aged care services
* Ensure compliance with aged care standards, government funding, and ASL services
* Provide clinical care expertise, including medications, wound management, and dementia/palliative support
* Complete all reporting including care plans in a timely manner, including maintaining accurate records and documentation
* Maintain knowledge of aged care regulations and CDC environments
* Utilise Microsoft Office Suite and scheduling software, prepare health metrics and general reports
* Oversee budgeting, supervision, and investigations
* Collaborate closely with elderly clients and their families
* Work a one-eighth rotating roster over one week from Monday - Monday
* Foster a positive culture aligned with ASL’s values of commitment, mission, respect, and honesty
About You:
* Collaborative and skilled leader with expertise in aged care, community services, and government funding, who values working closely with a hands-on leadership team.
* Completed or working towards a relevant industry qualification.
* Dedicated to optimising client outcomes by balancing needs, clinical care, and available services.
* Strong understanding of duty of care and reporting requirements in service delivery.
* Experienced in managing care plans, including risk assessments and liaising with external providers.
* Excellent interpersonal and communication skills, with a proven ability to lead with empathy.
* Skilled in coordinating rosters and leveraging IT systems for effective communication for remote teams.
* Adaptable to evolving aged care regulations and the Federal Government's Aged Care Roadmap.
* Committed to delivering high-quality care while navigating the complexities of aged care standards and funding models.
Essentials for working in Aged Care:
* Australian Working Rights
* Current and/or ongoing Flu and Hepatitis B Vaccinations
* Current unrestricted Australian’s Driver’s Licence and comprehensively insured car
Who You Are:
* AHPRA-registered Registered Nurse with proven leadership experience in clinical or aged care environments.
* Strong interpersonal and communication skills for effective family and team engagement.
* Ability to manage multiple priorities and oversee clinical care operations efficiently.
* Committed to maintaining high standards of care and compliance with aged care regulations.
* A solution-focused leader with a track record of managing teams and addressing clinical and staffing issues.
* A passion for delivering high-quality care and improving residents' wellbeing.
Willingness to:
* Obtain NDIS Workers Screen Check
* Undertake a pre-employment medical assessment
