The CNC Patient Flow sits within the Integrated Operations Centre as a part of the Patient Flow Team at the Launceston General Hospital and works collaboratively to ensure the right person receives the right care in the most appropriate setting.
The Role:
The CNC provides expert advice and leadership within an interdisciplinary team providing pathway development, patient assessment, care coordination and implementation of planned transfer of care to District Hospitals and Private Hospitals from within the clinical settings of the Launceston General Hospital (LGH) to enable care needs to be met in the most appropriate environment.
Details of appointment:
Permanent, part time, day worker position, working up to 16 hours per fortnight, commencing from the 22 June 2025. *notwithstanding hours to be negotiated with the successful applicant.
Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.
Salary: $114,851 to $121,886 per annum (Pro rata). Our Employer 11.5% superannuation contribution is on top of this amount.
* Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.
In addition, the following allowances calculated on the salaried incremental point may be available:
* Post Graduate Allowance of up to 10 % based on qualification type.

Successful applicants will be required to meet the essential criteria:
* Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
Desirable Criteria:
* Holds or is working towards a relevant post graduate tertiary qualification.
* Current Driver’s Licence.
