About the Role:
As a Community Care Worker, you will assist clients with general household tasks, day-to-day activities, personal hygiene and care based on their individual needs. You will be a part of a supportive team who have the ability to make a positive impact to older persons and improve their quality of life in the comfort of their homes.
Key Responsibilities:
Provide assistance with personal care tasks such as showering, grooming, dressing and skin checks.
Assist with medication support to prompt, administrator and supervisor medication.
Assist with tasks in your scope of practise as directed by the Registered Nurse
Engage In social interaction and provide companionship.
Transport clients to appointments and assist them during social outings.
Perform domestic duties such as vacuuming, mopping, dusting, laundry and meal preparation.
Meet with goals, needs and preferences outlined by the client and/or advocate.
Communicate, report and document client changes, deterioration and concerns to the Case Manager.
Participate in staff training, appraisals, and team meetings.
Contribute to quality improvement initiatives by gathering client feedback and adapting to workforce changes.
Be actively involved in an incident management system.
Essential Criteria:
A Certificate III in Individual Support or higher.
A full Australian driver's license with comprehensive insurance and use of own vehicle
Access to a reliable, registered, and insured motor vehicle.
Access to a smart mobile device.
Ability to work autonomously and adapt to the changing needs and schedules of our home care clients.
Desirable Skills:
First Aid & CPR, Manual Handling Certificate
Understanding of Italian culture and language.
Experience working with the elderly and knowledge of aged care issues
