Care Manager Lead
Ozcare is seeking a Care Manager Lead to support the development and growth of their Home Care Package business within a branch.
Your Role
* Provide leadership to a dedicated Care Manager team in planning, managing, and leading client care and support across various programs.
* Co-ordinate the involvement of Ozcare's multi-disciplinary teams and other services to ensure effective care.
* Ensure consistency in governance and compliance while driving service growth.
Key Responsibilities
* Manage and co-ordinate care services to ensure high standards of care.
* Develop and implement care plans tailored to individual client needs.
* Liaise with clients, families, and healthcare professionals to ensure comprehensive care.
* Foster strong relationships with clients, stakeholders, and team members to meet unique needs and goals.
* Ensure compliance with all relevant legislation and organisational policies.
* Monitor and evaluate care services to continuously improve quality and efficiency.
* Work closely with the Branch Manager and Care Managers to ensure service growth.
* Provide supervision and support to the Care Manager team.

Requirements
* Demonstrate experience in case management and client care coordination in a Community Aged Care setting.
* Show strong verbal, written, and interpersonal communication skills to work effectively with a broad range of people.
* Ability to develop staff, foster teamwork, and provide leadership.
* Provide proof of immunity or willingness to become vaccinated against MMR, Varicella, and Pertussis.
* Current First Aid and CPR Certificate.
* NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check).
* Current National Police Check prior to commencement.
* Hold a current Queensland driver's licence.
* Tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human Services).
