Working with various stakeholders, your role will focus on supporting customers from diverse backgrounds with complex support needs, enabling them to actively participate in their community. By collaborating with NDIS participants and their communities, you will ensure the delivery of tailored and individualised services that prioritise outcomes. Collaborate with stakeholders within a positive behavior support framework. Develop, implement, and review individualised behavior support plans. Conduct relevant assessments and gather data for behavior analysis. Meet customer acquisition and clinical activity targets.
