Position Profile:
The Clinical Nurse will provide effective management and facilitation of non-emergency patient transport ensuring timely, clinically appropriate and customer focused service provision. The Clinical Nurse will be the initial clinical escalation point for transport service providers to seek clinical advice and escalation. This will include liaison with key stakeholders including Government and Non-Government service providers. The position will develop and maintain effective links and collaboration with all Health Service Providers to facilitate the introduction of solutions as required.
ABOUT THE PATIENT TRANSPORT COORDINATION HUB (PaTCH)
The Patient Transport Coordination Hub (PaTCH) is an initiative of the State Health Operations Centre (SHOC) to coordinate bookings, scheduling and allocation of non-emergency patient transport in metropolitan WA. The aim of this centralised service is to improve the patient experience, minimise the use of emergency ambulances for non-emergency transport, enhance patient flow and improve the efficiency of our health system.
PaTCH aims to foster a culture of collaboration and transparency between transport providers and Health Service Providers (HSPs) that focusses on providing the best patient experience. It will do this by matching a patient’s transport requirements with the most suitable and available transport provider so that patients receive the most appropriate, safe and efficient transport based on their individual needs.
