About the role
Applications are sought from experienced Registered Nurses to join the Hospital in The Home (HITH) team at either RPAH or Concord Hospital. HITH is a hospital substitution program which means that the patient admitted to HITH would otherwise be accommodated in a hospital setting. HITH provides acute, comprehensive, and integrated clinical care in a clinic or home setting. The service operates 7 days per week, across the SLHD.
The CNS2 role includes responsibility for:
• Clinical operations, working in collaboration with multidisciplinary team to ensure efficient care delivery.
• Provide expert patient consultancy when delivering patient care.
• Liaise with internal and external stakeholders including hospital pharmacy departments, ED, RPA Virtual, Sydney District Nursing, Wound Command Centre.
• The CNS2 will attend meetings and committees relating to nursing practice and clinical management.
• Participate in quality improvement initiatives, research projects and the development and implementation of new clinical practices.
• Act as a resource to others in relation to clinical practice and service delivery
• Provide ongoing education to colleagues and supervision of student placements.
We are looking for someone who has …
1. A current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and post- graduate experience working with people in an acute, or community setting?
2. Excellent organisational, time management and interpersonal communication skills. In addition, a high-level proficiency in the use of computer software applications is essential for this position.
3. The ability to participate in a seven-day rotating roster (no night duty)
4. A current NSW Driver's Licence (P2 Licence acceptable)

About working for SLHD
All NSW Health workers are required to have completed a primary course (2 doses) of a Therapeutic Goods Administration (TGA) approved or recognised COVID-19 vaccine (except for the Janssen COVID-19 vaccine which is approved by the TGA as a single dose primary course).
