This position is responsible for the provision of culturally safe, patient centred services to Aboriginal clients and their families with chronic or complex needs by working with a culturally responsive and respectful Clinical Nurse Consultant in providing comprehensive assessment and care coordination along with education to clients, families and their carers that considers clinical, psychosocial, and cultural needs.Where You Will Be WorkingThis temporary full-time position is based in Inverell, on rich traditional Aboriginal lands, and provides outreach services to Ashford, Tingha, Glen Innes, Tenterfield, Armidale, Uralla, and Guyra.
