About the roleAs an Occupational Therapist, you will be an integral part of a community-based, multidisciplinary health team, supporting clients and carers in their homes to achieve optimal health independence.
Key responsibilities include:
Delivering comprehensive, coordinated, and integrated multidisciplinary services.Conducting Living at Home Assessments in collaboration with the Community Nurse.Providing client-centred care aligned with the principles of the Active Service Model for individuals from diverse backgrounds.Contributing to the development and delivery of high-quality, cost-effective community-based Occupational Therapy services.Meeting program and area health team key performance indicators (KPIs).Supervising and collaborating with City of Greater Geelong Allied Health Assistants in shared client care.About the cultureYou will have the opportunity to be part of a highly skilled, professional and dedicated team who take pride in providing high quality services.
