To provide clinical pharmacy services to the South Eastern Sydney Local Health District (SESLHD) to optimise safe and appropriate medication usage for patients and staff. The purpose of this role will be to promote best practice in the areas of Best Possible Medication History (BPMH), Medication Reconciliation (Med Rec) and Medication Review to optimise patient safety and outcomes.
The position forms part of a team led by a Team Leader Pharmacist and supported by other pharmacists and assistants/technicians. The position will be a district wide role, working across the SESLHD sites providing service with flexible hours across the seven day week, including one regular weekend shift as part of the medication management team and participating in on-call, afterhours, flexible work hours and public holiday work. It will be required to rotate through a variety of clinical and dispensary teams. The role willcollaborate with all SESLHD Pharmacy departments to ensure consistent and effective service provision across SESLHD.
SESLHD sites include, but are not limited to: Prince of Wales Hospital, Royal Hospital for Women, St George Hospital, The Sutherland Hospital and Sydney/Sydney Eye Hospital
