The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West and the Great Southern region of Western Australia. SWAMS delivers a wide range of primary health and community programs and has a strong growth strategy through partnership opportunities and future community development.
The position of NDIS Support Coordinator is responsible for the implementation and coordination of individualised supports and services for participants with a NDIS plan. This role will be vital in supporting and empowering people with disabilities to live in and access the community independently and to achieve their individual goals.
The role helps NDIS participants to understand and implement the funded supports in their plan and help link them to the community, mainstream and other government services that can assist in the implementation of their plan and achieving goals. The role will focus on supporting participants plan for the future, build on their strengths and skills, develop networks within their community as well as connect participants to providers. The role will assist participants to 'optimise' their NDIS plan ensuring they are getting the most out of their funded supports. The NDIS Support Coordinator is responsible for growing the Support Coordination services increasing the number of participants signed up to utilise Support Coordination through SWAMS.
Specific requirements of this position include but are not limited to:
Demonstrate your current working knowledge of the National Disability Insurance SchemeDemonstrated previous experience providing Support Coordination to NDIS participants and/or providing high level guidance to people with disabilities to identify and connect with support services to assist with their needsProvide details of your current knowledge of and proven ability to work and engage with the local Aboriginal CommunityProvide details to demonstrate a sound understanding of disability, mental health, challenges and best practice supportProvide details of current knowledge and proven ability to work with local community groups, services, agencies and government departmentsDetail your experience and skills managing multiple priorities and conflicting detailsProvide details of your computer skills including software packages used and capability with MS Word & MS ExcelApplicants of Aboriginal or Torres Strait Islander descent are encouraged to applyA certificate II, III or IV in Individual Support, Community Services, Aged Care, Mental Health or related fieldAll candidates must have a current WA Driver's License and will be required to undertake or have a Current Police Clearance and NDIS Worker Screening check prior to employment.
