About the Role
The Casey Social Work Department is looking for a Grade 1 social worker to join the team working in the subacute hospital setting including Gem and Rehab programs. This position is a full-time, permanent position and is part of a flexible team who collaborate to provide a holistic approach to service delivery across Casey Hospital, also working collaboratively across all sites bed-based and community.
* Showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psychosocial assessments.
* Development of patient centred care plans and complex discharges.
* Bed-based service delivery includes rosters servicing the Emergency Department, and experience in acute and women’s and children’s wards.
* You will be a member of a dynamic Social Work and Allied Health team working in a collaborative model of care across Monash Health.
* Work closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting.
Our Journey:
We are a busy and dynamic team assisting patients and families by providing a professional service for patients referred across the acute and sub-acute inpatient setting.
* Our Social Work service includes Acute, Sub-Acute (Aged and Rehabilitation) and specialty areas such as Intensive Care, Emergency Department and Women’s and Children’s services.
* This position will be based in our Social Work Subacute Team at Casey Hospital

You will be a member of a dynamic team with the opportunity to work in a setting assisting adult patient cohorts from general medicine, general surgery, Emergency department, Women’s and Children’s and other specialty areas as required. The focus is on psychosocial assessments, risk assessments, complex discharge planning and family support.

What you Need:
To be considered for this important role you will ideally possess the following:
* An AASW accredited Bachelor degree or entry level Master’s Degree of Social Work that provides eligibility for membership of the Australian Association of Social Workers
* Good organisational skills and a positive, enthusiastic attitude towards work
* Experience in working with culturally and linguistically diverse communities
* Able to work autonomously
* A friendly, outgoing and reliable demeanour
* Well-developed clinical assessment and complex discharge planning skills
* A current Victorian drivers licence
