About your opportunity:
In this position you will work at an advanced level with minimal supervision, working within a multidisciplinary health care setting to provide a high standard of service delivery to the Aboriginal and Torres Strait Islander community and its members in the maintenance and promotion of their health care and provide support across the Cassowary region in collaboration with other Cassowary Area Community Health multidisciplinary staff.
About your role:
5. To be self-directed and show initiative under the guidance of the Team Leader, NG 7 Clinical Nurse Consultant (CNC), and Aboriginal and Torres Strait Islander Health Worker Cluster Coordinator.
6. Work under professional and clinical supervision of the Team Leader, CNC, Registered Nurse, and Aboriginal and Torres Strait Islander Health Worker Cluster Coordinator, participating in case review and engaging in professional and performance development activities, in-service training and ongoing learning and development.
7. Provide and participate in effective and efficient comprehensive care coordination, care planning and evaluation, and quality improvement process which includes administrative duties including maintaining data.
8. Deliver advanced Health education, promotion and screening activities according to the needs of Aboriginal and Torres Strait Islander communities in a confidential and non-judgemental manner using culturally acceptable strategies and evidence-based practice.
