Our services create intentional avenues for young people to explore opportunities, build support networks, and access, participate, and contribute to their communities. Youth services provide a diverse range of programs which include housing and homelessness, driver training, education, employment and training, AOD support, social and community activities, specialist therapeutic responses, and youth justice programs. About the role We are seeking a values-driven and proactive Case Manager, at our Surry Hills location. Reporting to the Team Leader, this is a permanent, full-time position.Salary and conditions are in accordance with the SCHADS Crisis Award, level 2. You will successfully Establish effective relationships with clients that encourage self-determination and participation.Provide case management support to clients in accordance with TSA case management models and standards of practice.Ongoingly assess the client's issues, needs, and circumstances from initial intake to exit.Ensure Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients.Ensure case plans are implemented with active client participation.Ensure clients are assisted to access support services and establish community supports, and where needed are assisted through advocacy with service providers.Monitor progress and outcomes of individual case plans are regularly monitored and reviewed and where needed action is taken to ensure the plan continues to meet client needs or goals. You will have (Requisite Skills & Qualifications) Degree qualification in social work or related field preferred. Minimum of Diploma level qualification in a relevant discipline - Youth Work, Community Services / Case Management.Relevant experience of case management in a Youth work and/or social service environment.A strong commitment to working within a trauma and psychologically informed environment.Ability to create a culturally safe and inclusive workplace/Crisis accommodation.Experience in working with young people in crisis intervention & homelessness. Demonstrated knowledge of the causes of youth homelessness and how to assist young people to work toward long term sustainable housing.Demonstrated experience in building and maintaining collaborative relationships with internal and external providers.Ability to respond immediately and effectively to unsafe situations on site including client critical incidents.Capacity to manage time & competing demands in a high volume crisis setting, whilst maintaining record keeping and reporting requirements.Current NSW Driver's License.Senior First Aid certificate.Valid NSW Working with Children Check.
