The Role
The Transitional/Nurse Practitioner (TNP/NP) - Palliative & Aged Care is a state-wide position and is responsible for collaboratively planning, providing, monitoring and evaluating the delivery of specialist Palliative and Aged Care Nursing across the Justice Health and Forensic Mental Health Network (Justice Health NSW) in conjunction with the Aged Care Nurse Practitioner. The TNP/NP will achieve this by working internally with primary care, specialist and sub- acute care teams and services.
The TNP/NP - Palliative & Aged Care will collaborate with external care teams as indicated to plan and provide a collaborative shared care service to patients, and to ensure continuity of care when transferring between locations or for patient release.
The TNP/NP – Palliative and Aged Care utilises critical and reflective thinking, complex reasoning and analysis to guide assessments, clinical judgements, diagnosis, therapeutic management and decisions for complete episodes of person centred care.
The TNP/NP – Palliative & Aged Care is clinically supervised by the Justice Health NSW Geriatrician Staff Specialist, Aged Care NP and Palliative Care Staff Specialist and has substantial autonomy in the management of patients.
