selectability, is currently seeking a dedicated TCP (Transition Care Program) Case Manager to provide exceptional support and coordination for our clients.
In this vital role, you will work closely with clients, families, and healthcare professionals to ensure smooth transitions and the delivery of high-quality care.
Key responsibilities include but are not limited to: lead and collaborate with interdisciplinary teams to ensure the effective implementation of program objectives provide guidance and supervision to program staff to ensure the highest standards of care and service delivery develop and implement comprehensive therapeutic care plans for clients, considering their unique needs and circumstances conduct consultations with community services to enhance collaboration and support networks for clients maintain accurate and up-to-date documentation and clinical records in accordance with regulatory standards facilitate smooth and well-coordinated discharge planning in collaboration with healthcare professionals, ensuring continuity of care for clients post-transition provide support to clients and their families to address the emotional aspects of the transition period deliver education sessions to clients and families, promoting health literacy and empowering them to actively participate in their care offer ongoing support services to enhance the overall well-being of clients throughout the transition period About you To be considered for this rewarding role you will have: excellent communication and interpersonal skills.

previous experience in program leadership and case management strong understanding of therapeutic care planning and discharge planning processes ability to work collaboratively in a multidisciplinary team effective prioritisation and time management skills excellent written and verbal communication skills Role Requirements travel to selectability sites will be required AHPRA Registration a current minimum P1 "C" class Queensland drivers licence a currently registered, roadworthy, and fully comprehensive insured vehicle a Positive Notice Working with Children (blue card) (or the ability to obtain prior to commencement) a current yellow card exemption or an NDIS Worker Screening Clearance (or the ability to obtain prior to commencement) NDIS Worker Orientation Module 'Quality, Safety & You' completion certificate About the offer This is a XX-time role based in Townsville.
