Target Business Services Location: The Ponds NSW 2769 Salary: $73,500 plus 11.5% Superannuation Position: Full time - Permanent Our client, Australian National Care is a registered aged care/ disability services and NDIS provider providing high-quality care and support services to vulnerable members of community. The client has a dignified support system of experts and offers a wide range of services that can be customized depending on the needs and requirements. The client is looking for a Disabilities Services Officer who can live by its organisational values and has a genuine desire to make a difference in a customer focused model of care with integrity and innovation. The role has a strong focus on providing comprehensive, coordinated and integrated services to clients. Your main task is to support, provide education and community access to people with intellectual, physical, social and emotional disabilities. You will be accountable to support people with disability, their families and carers to access other health and human services to manage other presenting issues (for example advocacy with the National Disability Insurance Scheme and support to liaise with care agencies due to changes in circumstances or support to access accessible accommodation if required). The ideal candidate would be a self-motived individual that possesses knowledge and understanding of the NDIS and working with people with Disabilities. Minimum Requirements: Relevant diploma with specialisation in Disability Services with two years of full-time work experience in Community or Disability Services. The successful candidate will be responsible to: • Assist individuals to identify appropriate and achievable goals as well as strategies to achieve these, ensuring dignity, privacy and confidentiality at all times. • Participate in developing and monitoring individual plans and be accountable for their outcomes. • Assist in the delivery of programs that facilitate community inclusion and participation, promoting individual needs, abilities and preferences. • Interview clients and assess the nature and extent of difficulties. • Monitor and report on the progress of clients. • Refer clients to agencies that can provide additional help. • Work effectively with people who have high physical needs and/or behaviours of concern implementing manual handling assessments and/or behaviour support plans as required and in keeping with company procedures and policies. • Promote community inclusion through actively seeking community-based opportunities for individuals based on their particular interests, needs and cultural requirements. • Prepare submissions for funding and resources, and reports to government bodies and other agencies • Provide high quality support that addresses client needs as articulated in their Plan and enhance their independence, abilities, community participation and quality of life. • Respect the personal preferences of clients in meeting their needs and encourage individual growth and development. • Work in conjunction with other Support Workers and management to perform duties as directed. • Adhere to budgets, administrative and data collection ensuring all procedural requirements are met in a timely manner. • Maintain confidential client, personnel and organisational information in line with legislative requirements and company policies and procedures. • Comply with Federal, NSW and Local Government legislation, regulations, permits and/or by laws. • Comply with Service Agreements, operational guidelines, approved work plans, reporting requirements or task directives. • Work collaboratively with the staff and management to address service improvement requirements resulting from client complaints, stakeholder feedback and/or internal or external evaluation processes. • Participate in organisational communications and development systems such as email, staff meetings, planning & review days, quality assurance and organisational promotions and events as required. • Ensure that all programs and service delivery are linguistically and culturally appropriate to the needs of the people we support. • Maintain documentation, records and data collection as required and in accordance with the relevant legislation and company policies. Experience, Skills and Knowledge • Preferably experience in working within an Individual Planning process for people with disability or within the context of the National Disability insurance Scheme (NDIS) and experience in community services sector is highly desirable. • Demonstrated ability to respond professionally and effectively to competing priorities, use initiative and work independently and in a team environment. • Good verbal and written communication skills including the demonstrated ability to work independently and report accurately to supervisor. • Demonstrated interpersonal skills including the ability to work and communicate effectively with participants, their families and community agencies. • Proficiency in the use of computer software such as Word and Excel. • Demonstrated willingness to participate in supervision, reflect on practices, review performance and adhere to work plans and reasonable workplace directives. • Ability to display discretion and sound judgment, with confidential and sensitive information. • A commitment to non-discriminatory practice base, WHS and access and equity principles.
