NDIS Support Coordinator – Brisbane Northern and Southern regions. YES Service YES Service (YESS) is an independent registered National Disability Insurance Service (NDIS) provider specialising in the delivery of Support Coordination services.
YESS works closely with NDIS participants with a wide range of disabilities and psychosocial needs. We assist people to recognise their value, strengths and goals. We place a high emphasis upon helping a participant to get the most out of their NDIS experience while making it easy for them to maintain or improve their health and wellbeing, social participation and independence as they grow their meaningful relationships with those significant to them in their life.
ABOUT THE OPPORTUNITY Due to our continued growth in Queensland, we are looking to recruit a Support Coordinator to assist our clients who require services provided by their NDIS plans. You will be working with clients in the Brisbane's Northern Suburbs and toward the Sunshine Coast. You will be working from your home office. We are offering a Full-time position.
You will be required to utilise your extensive knowledge of the NDIS to deliver timely and effective support coordination services to a caseload of NDIS participants to ensure they are receiving the suitable independent options and they are connected in a way that supports their goals. Some of your responsibilities will include:
Provide Support Coordination services to NDIS Participants working independently and as part of a team. Assist clients, families and carers to connect with services both mainstream and within disabilities.Assist during times of crisis with the ability to prioritise your tasks.Accurately record all progress and agreed outcomes and actions including timely reports to the NDIS on the progress of a participant's plan. Respond to new support co-ordination enquiries for new participants.Work with new participants to help establish and implement a Support Coordination plan that focuses on the participant achieving their plan goals.Build up a reliable resource base of service providers across the local area. ABOUT YOUThe role of a Support Coordinator is pivotal for participants of the NDIS and to be successful in this role, we are looking for someone with the following:
Strong applied knowledge of the NDIS and it's service sector. High emotional intelligenceStrong administration and record keeping skills.Exceptional time-management with an ability to manage and deliver to multiple deadlines.Ability to work with a caseload in a billable hours environment.Tertiary qualifications in disability or community services, or substantial experience working within the NDIS framework. Clear, effective communication skills - both written and verbalHighly computer literateAbility to work autonomously as part of an overall team, from a home office workspace.Be a self-starter, with a high degree of problem-solving skills.Current Driver's License and the ability to travel to visit clients/services as required.
