Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.The RoleWe are seeking an energetic Social Worker to join our vibrant and supportive Social Work team at Caulfield Hospital working in the sub-acute environment. You will have relevant hospital experience and be able to work within a strong multidisciplinary team. To be successful you will exhibit an understanding of social work theories and interventions and have excellent interpersonal, communication, and counselling skills that enable you to provide a high-quality counselling, support, and discharge planning service to patients and their families. A sound understanding of complex discharge planning within the hospital setting, including an understanding of the Transition Care Program, My Aged Care, VCAT, and the NDIS is important. The successful candidate will need the professional skills to collaborate with patients, families, other disciplines, and community stakeholders, and this position will provide the opportunity to extend these skills. This is a fast-paced environment and requires timely and quality discharge planning. Comprehensive orientation, training, clinical supervision, and peer support are available for this role to support the successful candidate to work flexibly, creatively, and collaboratively.
