A new & exciting opportunity is now available for an experienced Occupational Therapist to join the team at Bolton Clarke due to growth in the team. Based out of our Bundoora office in Melbourne's NorthThis is a part role (48 hours a fortnight) or Full time position with the ability to work from home. This home & community care role will provide clinical support services to our valued clients in their own homes, enabling them to stay at home & remain independent for as long as possible. All the tools of the trade will be provided, including a company smart phone & laptop. The ideal candidate will possess exceptional analytical, problem solving & negotiation skills with the ability to seamlessly manage varying and/or conflicting stakeholder demands. Duties: Conduct home based assessments including all areas of activities (daily living, mobility & home safety). A focus on falls prevention & contribute to the continuous improvement to the overall program. Develop care plans in consultation with the resident & their family to ensure we are providing the highest quality of care for each & every client. Work to foster productive working relationships with relevant government departments, hospitals, key providers & other specialist providers including network with occupational therapy services & other practitioners within the region. Previous relevant experience within an Aged or Community care environment. Strong understanding of all relevant legislative, regulatory requirements, professional standards & guidelines relevant to the Aged Care sector (desirable). Qualifications: Bachelor's degree in occupational therapy or equivalent. Current Occupational Therapist Registration with AHPRA. Benefits: Work with a dedicated & supportive clinical care team.
