About the Role:
As a Clinical Care Coordinator, you will lead clinical and care teams to ensure residents receive exceptional care, understanding their unique identities and experiences. You will also play a pivotal role in our home's clinical governance and may manage specialized clinical portfolios, such as education, palliative care, and infection control.
Key Responsibilities:
* Provide comprehensive clinical and specialized nursing care tailored to each resident's needs.
* Prioritize residents and families in decision-making, considering preferences, independence, dignity, and identified care-related risks.
* Oversee scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required.
* Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly.
* Conduct routine clinical assessments to address identified gaps in care.
About You:
* Currently registered as a Nurse with AHPRA, with no restrictions.
* Demonstrated senior management experience in aged care.
* In-depth knowledge of the aged care industry, with an eye for emerging trends and best practices.
* Proficiency in managing human resource matters, complemented by strong negotiation skills.
* Skilled in utilizing various information technology tools and software.
