Job Description
The Primary Health Registered Nurse and an opportunity for team leading is responsible for overseeing the delivery of primary healthcare services within the organisation. This role involves contributing to a multidisciplinary team, ensuring high-quality patient care, and promoting health equity in the community. The will drive program development, enhance team performance, and collaborate with stakeholders to address the diverse health needs of the population.
Key Responsibilities:
* Program Management: Develop, implement, and evaluate primary health programs and services, ensuring they are evidence-based and responsive to community health needs.
* Patient Advocacy: Serve as a key point of contact for patients, advocating for their healthcare needs and ensuring they receive comprehensive, coordinated care.
* Quality Improvement: Monitor and assess the quality of care provided, using data and feedback to identify areas for improvement and implement necessary changes.
* Collaboration: Work closely with healthcare providers, community organizations, and stakeholders to enhance service delivery and expand access to care.
* Education and Training: Provide ongoing training and support to team members on best practices, health promotion, and patient engagement strategies.

Qualifications
* Education: Bachelor's degree in nursing, Public Health, or a related field; master's degree preferred. Current AHPRA accreditation
* Experience: Minimum of 5 years of experience in primary healthcare

* Knowledge of primary healthcare practices and regulations.
