You’ll work within a multidisciplinary team to deliver patient-centered healthcare, manage health risks, and support rehabilitation. This is a full-time, permanent role based in Wooroloo, WA. As a Registered Nurse in the Primary Care team, you’ll make a positive impact by: Conducting clinical assessments, care planning, and discharge coordination. Providing education, counseling, and health promotion services. Collaborating with healthcare professionals to ensure quality patient outcomes. Maintaining accurate clinical documentation in line with facility standards. Supporting and orienting new staff members and students. Contributing to quality improvement initiatives and policy development.
