Clinical Care Coordinator
Lead the clinical team to deliver excellent care and improve quality of life for residents.
Responsibilities:
* Supervise care services, ensuring safety, effectiveness, and regulatory standards.
* Lead, mentor, and support the nursing team, including managing schedules, conducting performance reviews, and fostering a positive work environment.
* Develop and review individualised care plans, collaborate with multidisciplinary teams, and address changes in resident needs promptly.
* Participate in quality assurance activities, conduct audits, and implement improvements to maintain high care standards.
* Ensure accurate and timely documentation of care plans, incidents, and resident records.
Key Requirements:
* Prior experience as a Clinical Care Coordinator (or similar leadership position) is highly preferable.
* Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
* Excellent clinical knowledge, leadership, organisational, and communicative skills with the ability to lead, manage and motivate a clinical team effectively.
* Strong knowledge and experience of aged care accreditation, regulations, standards, and best practices.
