Your opportunity:
* To provide expert clinical and patient-centred care within Integrated Care Services by supporting the facilitation and provision of quality and cost effective nursing services which are consistent with contemporary standards and evidence-informed practice. The Clinical Nurse (Discharge Planner) supports the Nurse Unit Manager and other members of the Integrated Care Services team with the timely transitions of care through the patient journey and the provision of clinical leadership, advice and advocacy to a multidisciplinary team.

Our offer:
You will receive a competitive remuneration package value up to $65,950 p.a., comprising salary between $54.45 - $58.30 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)) (Permanent Part Time, 38 hours p.f.)
