Position Summary
The Physical Therapist provides direct rehabilitation care to patients upon physician referral, in accordance with applicable laws and regulations.
Education, Experience, and Licensure Requirements
* Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT) from 2003 onwards must have a Master's or Doctoral degree.
* Currently registered/licensed in the state. Must maintain an active license throughout employment.
* Prior experience in post-acute care or related settings is preferred.
* CPR certification is required upon hire or obtained during orientation.
Specific Job Requirements
* Participate in community health matters/projects as appropriate.
* Proficient in Microsoft Word, Excel, and communication tools.
* Liaise with patients, families, and support departments to plan patient care.
* Maintain good body mechanics at all times.
* Makes independent decisions when circumstances warrant such action.
* Familiarity with practices, procedures, laws, regulations, and guidelines governing functions in the post-acute care facility.
* Implement and interpret department programs, goals, objectives, policies, and procedures.
* Perform competently in areas including patient rights, safety, and sanitation.
* Maintain professional relationships with associates, vendors, etc.
* Maintain confidentiality of proprietary and confidential information.
* Understand and follow company policies, including harassment and compliance procedures.
* Demonstrate integrity and professionalism by adhering to Life Care's Code of Conduct and completing mandatory training.
Essential Functions
* Follow physical therapy treatment plans for patients.
* Establish, assess, and modify realistic, measurable, timely, and functional patient goals.
* Oversight and evaluation of care given by PTAs, Rehab Aides, and students.
* Chart appropriately and timely.
* Utilize therapy software accurately.
* Exhibit excellent customer service and a positive attitude towards patients.
* Assist in patient evacuation.
* Demonstrate dependable attendance.
* Concentrate and use reasoning skills and good judgment.
* Communicate effectively on an interdisciplinary team.
* Mobility and physical stamina to perform tasks intermittently during working hours.
* Read, write, speak, and understand English fluently.
