About the role
We are seeking a values-driven and proactive Living and Life Skills Worker to support and guide young people to identify barriers to independence and support the development of practical life-skills and social skills to transition to independent living in the community. This role aims to improve young people's level of engagement with family, friends, education/employment, and the local community to develop capacity to maintain their general health and well-being.
Reporting to the After Hours Client Services Coordinator, this is permanent full-time position, based in Shepparton, VIC. Salary and conditions are in accordance with the SCHADS Award Level 5.
You will successfully
* Undertake case management with clients utilising case management frameworks and tools as per TSA policy and procedure
* Maintain partnership arrangements, building service relations and inter-agency relationships
* Ensure accurate, up to date client records and data are maintained for all clients in accordance with TSA standards using TSA information systems
* Participate in client feedback sessions and completion of outcome reports as required by the agency and funding bodies
* Identify risks to clients, staff, and the organisation and reporting these to management as soon as possible
You will have
* Tertiary qualification in social work, welfare, community development or related field
* Relevant experience in a social service environment preferably the youth sector
* A current and valid Working with Children's Check
* A current and valid Victorian Driver's Licence
