Senior Social Worker - Transitional Care Coordinator Fixed term temporary full-time opportunity to deliver high-quality evidence-based Social Work clinical services and leadership in the assessment and treatment of consumers with complex mental health issues, their families, carers, and relevant stakeholders, in an acute and post-acute setting. This position operates as a member of a multidisciplinary mental health team with a focus on planning and providing interventions within a therapeutic recovery framework. About you Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values: Apply knowledge of mental health legislation and policy (including Mental Health Act 2016 and the National Mental Health Standards) to facilitate the delivery of coordinated and integrated services within Metro North Mental Health, in partnership with emergency services. Practice collaboratively within the interagency health care team to promote the safety, security, and personal integrity of individuals and groups. Support improvements in service delivery by identifying inconsistencies between practice and policy, and work in collaboration with the interagency team/s to implement changes.
